You must be a client at our clinic and your pet must be a have been examined within a year to establish a VCPR (Veterinary Client Patient Relationship) to have medication prescribed.
Your veterinarian may request to see your pet before filling the medication.
Your pet must have been prescibed this medication once before.

This form is for longterm medications only, not antibiotics or ear medication.
